A square sheet of paper is folded twice and then a single hole is punched through the folded paper, as shown below:
Folding sequence:
- Fold the paper in half from left to right (so the left edge meets the right).
- Then fold the resulting rectangle in half from top to bottom (so the top edge meets the bottom).
- Punch one hole near the center of the folded paper — but slightly offset toward the right (see diagram).
