Folding sequence:

  1. Fold the paper in half from left to right (so the left edge meets the right).
  2. Then fold the resulting rectangle in half from top to bottom (so the top edge meets the bottom).
  3. Punch one hole near the center of the folded paper — but slightly offset toward the right (see diagram).
